Commissioned by the ICB, it will provide low-intensity, holistic support for working-age people (16+) with a health condition or disability that is affecting their work, \u00a0whether they need help to stay in work, return from sickness absence.<\/p>\n<p style=\"font-weight: 400;\">\n<p style=\"font-weight: 400;\">Each participant will receive a personalised assessment from a Work and Health Coach, backed by a multidisciplinary clinical team, and a tailored plan that can cover health support, workplace adjustments, employer liaison, and wider issues such as finances or housing.
